Kindergarten registration make-up day scheduled for March 31

On Thursday, March 31, 2022 Hubbard Elementary School will conduct a make-up day

for Kindergarten Registration for the 2022-2023 school year by APPOINTMENT ONLY.

Please call 330-534-1921 ext. 4001 to make an appointment.


Parents/guardians will come to the elementary school WITH THEIR CHILD at their

scheduled time. Upon arrival, the children will attend a brief screening and parents will

complete the online registration while the children attend a brief screening. Laptops will

be provided and we will have assistance available.


Parents/guardians MUST MAKE A COPY of the items listed below for the

school to keep. Final registration will NOT occur unless ALL COPIES ARE

PROVIDED.


  • Child’s birth certificate (Must be 5 years of age before September 30, 2022)

  • Immunization records

  • Proof of residency which includes ONE of the following four examples:

    •  Mortgage/Bank Payment Book with name and addresses

    •  Lease (notarized) AND water bill with resident’s name and address

    •  Most current unpaid gas, phone, electric bills with name and address AND two pieces of business mail in the stamped envelope

    •  Municipal Income Tax Registration

  • Custody or guardianship records (if applicable). Documentation is needed in those cases dealing with divorced parents or a court ordered guardianship.

  • Photo ID of parent/guardian. One of the three examples below is required:

    • Ohio Driver’s License

    • Ohio I.D. card

    • Some other valid type of photo I.D.
